What Is an N-648 Evaluation?
An N-648 evaluation, formally known as a Medical Certification for Disability Exceptions, is a specialized assessment required during the U.S. naturalization process. This form is used when an immigrant is seeking an exemption from the English language and/or civics test based on a documented physical, developmental, or mental impairment.
The purpose of the evaluation is to determine whether a medical condition prevents the individual from fulfilling the testing requirements necessary for U.S. citizenship.
The Role of a Psychologist in the N-648 Process
Psychologists play a vital role in completing N-648 evaluations, especially when the exemption request is related to a cognitive, emotional, or psychiatric condition. Their responsibilities include:
Assessment of Impairment
The psychologist conducts a comprehensive clinical evaluation to assess whether the individual meets the criteria for a cognitive, developmental, or psychiatric disability. This may include clinical interviews, psychological testing, and reviews of existing medical records.
Diagnosis and Documentation
Based on the evaluation, the psychologist provides a formal diagnosis. This diagnosis must clearly explain how the condition affects the person’s ability to learn or demonstrate knowledge of English and/or U.S. civics.
Completion of the N-648 Form
The psychologist fills out the N-648 form with clear and accurate information about the applicant’s condition, medical history, and how the impairment affects learning and information processing. Each section must meet specific USCIS documentation standards.

Psychologists with experience in clinical, developmental, or neuropsychological assessment are especially qualified for these evaluations. Their training allows them to accurately assess complex conditions and provide thorough documentation that supports the exemption request.
Collaboration and Advocacy

How to Schedule an N-648 Evaluation with Us
If you are interested in getting an N-648 evaluation through our practice, here are the steps to get started:
- Text us at 267-209-3209 and let us know you are interested in scheduling an N-648 evaluation.
- In your message, please include your full name, date of birth, and email address.
- Before we can schedule your appointment, payment must be received via Cash App, Zelle, or credit/debit card.
- Once payment is confirmed, we will schedule your appointment at a time that works for you. You will also receive access to a secure portal where you will complete a few assessments before your appointment.
- The evaluation session lasts approximately one hour and is typically conducted through video conferencing. During the appointment, the psychologist will assess your cognitive and emotional functioning.
- After the evaluation, the N-648 form will be completed and sent to you or directly to your immigration attorney.
